HS to host student vaccination clinic for ages 12+
ConnextCare has partnered with the Oswego County Health Department to hold a vaccine clinic for students aged 12+ who attend the Mexico Academy and Central School District.
The clinic will take place in the High School gymnasium on Friday, May 21, 2021 with appointments being offered 12:30-2:30 p.m. and 4-6 p.m. There are two links below for parents to fill out for the clinic.

In addition to the state registration link, parents/guardians must complete the second link below that is specific to ConnextCare and contains information it needs to administer the vaccine. This form allows for parents/guardians to consent for vaccine administration during the school day and without them present, if they choose. Please note that ConnextCare will be collecting insurance information and billing an administration fee for the service.
The clinic asks that registration forms be completed by Wednesday, May 19.
