How do I refer my child for an evaluation?

A: If you feel your child needs to be evaluated to determine if they qualify for Special Education services, please send us a letter requesting an evaluation (see address below) with a short description of your concerns about your child.

Once we receive your written request for an evaluation, we will mail you a packet consisting of:

  1. Initial Evaluation Consent Form
  2. Parent Guide to Special Education
  3. Procedural Safeguards Notice
  4. Social History Form

When we receive the signed consent form and completed Social History, the process of evaluating your child begins. Once the evaluation(s) have been completed, a CSE (Committee on Special Education) meeting will be held to discuss the results of the evaluation(s). If your child is found to be eligible for Special Education Services, an IEP (Individualized Education Program) will be developed outlining programs and/or related services to accommodate your child’s needs. The IEP will be reviewed annually.
